Pre-registration is not a requirement, but it is highly recommended.
Also, everyone who pre-registers helps the convention host committee by providing a more accurate attendee
count which will be used for planning purposes for lunch, acquiring prizes, etc.

Print a copy of the above form, fill it out, and mail with your fee(s) to:

A larger, clean copy of the registration form is also available by downloading and printing a .pdf version of the CVA Convention Registration Form Note, it is suggested that you first download the file to your computer, then open it using the Acrobat Reader Software. If you don't have the Acrobat Reader Software, you may obtain a free copy from Adobe.
